LIABILITY WAIVER & RELEASE OF CLAIMS
Assumption of Risk
I acknowledge that participation in indoor playground and café activities involves inherent risks including, but not limited
to, slips, falls, collisions, equipment failure, and other hazards that may result in injury, illness, or death. I voluntarily
assume all such risks on behalf of myself and my child(ren).
Parental Supervision Requirement
I understand and agree that this facility is not a daycare. I accept full responsibility for the supervision, behavior, and
safety of my child(ren) at all times while on the premises.
Release of Liability (INCLUDING NEGLIGENCE)
To the fullest extent permitted under South Carolina law, I hereby release, waive, and discharge Sip and Play Cafe LLC,
its owners, officers, employees, agents, and affiliates from any and all claims, demands, causes of action, damages, or
liability arising out of or related to participation in activities at the facility, INCLUDING ANY CLAIMS ARISING FROM
THE NEGLIGENCE OF THE RELEASED PARTIES.
Indemnification
I agree to defend, indemnify, and hold harmless Sip and Play Cafe LLC from any claims, damages, costs, or expenses
(including attorney’s fees) arising from my or my child’s actions or participation.
Rules & Safety Compliance
I agree to follow all posted rules, including but not limited to: socks required, no shoes in play areas, no food or drinks in
play zones, and adherence to age-appropriate areas. I understand failure to follow rules may result in removal without
refund.
Health Representation
I certify that I and my child(ren) are in good health and free from contagious illness. I agree not to enter the facility if
experiencing symptoms of illness.
Photo & Media Release
I grant permission for photographs or video taken on the premises to be used for marketing purposes unless I provide
written notice declining consent.
Severability & Governing Law
If any portion of this agreement is found unenforceable, the remaining provisions shall remain in full force. This
agreement shall be governed by the laws of the State of South Carolina.
